Schedule Change Request Form
We are so excited to welcome you back to Lakeside for another great year! Please understand that at this point schedules are fluid and will be changing.  Our goal is to make sure every student has a complete schedule on the first day of school.

We will only be granting schedule changes for the following reasons:
1. A scheduling conflict resulting in a blank spot during a period.
2. A class that has already been taken and passed, including through summer school.
3. A missing class needed for graduation requirements.
4. A class that you did not request (We have the requests).
